Maintenance request are accepted on-line, please click on the link above to make your request. To ensure proper handling of your request please make sure to include a phone number and your current address on the request. You will be contacted within 1 business day of submition of your request. You may also fax your written request to 614-231-4653 or mail/drop off at 573 Carle Ave, Lewis Center, OH 43035.
